ILD/Pneumonitis is the most critical adverse event of special interest. Strict protocol adherence is essential:
Grade 1 (Asymptomatic radiographic findings): Interrupt T-DXd immediately; initiate systemic corticosteroids (Prednisolone 0.5 mg/kg/day); resume only after complete radiological resolution.
Grade 2+ (Symptomatic): Permanently discontinue T-DXd; administer high-dose corticosteroids (Prednisolone 1–2 mg/kg/day or IV Methylprednisolone).
